Levels of care
The right care for every moment
Needs change along the way — and hospice is designed to change with them. Medicare defines four levels of hospice care, and Care Plus provides all of them, adjusting seamlessly as the situation calls for it.
Routine Home Care
The most common level of hospice. Our team visits regularly wherever the patient lives — home, assisted living, or a facility — managing symptoms and supporting the family, with a nurse on call 24/7.
Continuous Home Care
When symptoms flare and need close, hands-on management, we provide extended nursing care in the home to bring things back under control — helping families avoid a hospital trip.
Respite Care
Caregiving is demanding. Respite care provides short-term care in a facility so family caregivers can rest and recharge, knowing their loved one is in caring hands.
General Inpatient Care
For symptoms that can’t be managed at home, short-term inpatient care provides round-the-clock clinical attention until comfort is restored and the patient can return home.
