01 · About this policy

This Privacy Policy explains how Care Plus Hospice Care (“Care Plus,” “we,” “us”) collects, uses, and safeguards personal information you provide through our website, azcareplus.com (the “Site”). It also explains the choices and rights you have regarding that information.

This Privacy Policy is separate from our Notice of Privacy Practices under HIPAA, which governs how we handle protected health information (PHI) about our hospice patients. If you are looking for how we handle patient medical records or billing information, please see that Notice instead.

By using this Site, you agree to the collection and use of information in accordance with this Privacy Policy.

02 · Information we collect

Information you provide directly

When you contact us through the Site — for example, by submitting a contact form or emailing our office — we collect only the details you choose to share, such as your name, phone number, email address, ZIP code, and the message or question you send us.

Please do not submit protected health information (PHI) or other sensitive medical details through the website form. Those conversations should happen directly with our clinical team by phone or in person.

Information collected automatically

When you visit the Site, we and our service providers automatically collect limited technical information, including:

  • IP address (often anonymized or truncated);
  • Browser type, device type, and operating system;
  • Referring URL (the page that led you to our Site);
  • Pages viewed, time spent, and navigation paths;
  • Approximate geographic location at the city or region level.

Information from cookies and similar technologies

We use a minimal set of cookies to keep the Site functioning and to understand how visitors use it. See Section 05 for details on cookies and your choices about them.

03 · How we use your information

We use the information we collect for these purposes:

  • To respond to your inquiry and provide the information or services you requested;
  • To coordinate hospice-related conversations with your family or referring physician when you have asked us to do so;
  • To improve the Site’s design, content, and performance;
  • To understand which pages and services are most useful to visitors;
  • To detect, prevent, and respond to security issues, fraud, or misuse of the Site;
  • To comply with applicable law, respond to lawful requests, or protect the safety of our staff or others.

We do not sell your personal information. We do not use your personal information for third-party advertising or marketing unrelated to hospice services.

04 · How we share your information

We share personal information only in the limited circumstances below:

  • With members of our care team — nurses, physicians, aides, social workers, chaplains, and other clinicians directly involved in responding to your inquiry.
  • With trusted service providers who help us operate the Site or deliver our services (web hosting, email delivery, analytics), each bound by written confidentiality obligations.
  • As required by law — including subpoenas, court orders, and requests from regulatory or law-enforcement authorities acting within their authority.
  • In connection with a business transfer — if Care Plus Hospice is acquired by or merged with another organization, personal information may be transferred as part of that transaction, subject to the same privacy protections.
  • With your consent — for any other purpose disclosed to you at the time of collection.

Care Plus Hospice does not sell personal information to third parties and does not participate in cross-context behavioral advertising.

05 · Cookies, analytics & third parties

What cookies are

Cookies are small text files a website stores on your device to help it function properly and understand how visitors use it. Some cookies are erased when you close your browser (session cookies); others remain until they expire or you delete them (persistent cookies).

Cookies we use

  • Strictly necessary cookies — required for the Site to work properly (for example, to preserve your form entries during a session).
  • Analytics cookies — help us understand which pages visitors find most useful. Analytics data is aggregated; IP addresses are anonymized where technically supported, and we do not use cross-site tracking.

Third-party services

Our Site may load resources from a small number of third-party providers:

  • Google Fonts (typography loading);
  • A basic web analytics service (aggregate visitor traffic).

We do not use third-party advertising cookies, retargeting pixels, or social-media tracking tags.

Your cookie choices

You can control cookies through your browser settings. You may block or delete cookies without losing access to any of the Site’s core information. Where feasible, we honor browser “Do Not Track” (DNT) signals and Global Privacy Control (GPC) signals as opt-outs from analytics.

06 · Your rights & Arizona disclosures

Regardless of where you live, Care Plus Hospice voluntarily honors the following rights for anyone who contacts us through the Site:

  • Right to know what information we hold about you;
  • Right to request correction of inaccurate personal information;
  • Right to request deletion of personal information we hold about you (subject to legal and business retention requirements);
  • Right to opt out of marketing communications at any time;
  • Right to withdraw consent for any use of your information that requires it.

To exercise any of these rights, contact us using the details in Section 09. We will respond within 30 days.

Arizona residents

Arizona has not enacted a general consumer data privacy statute as of this policy’s effective date. However, Care Plus Hospice voluntarily provides Arizona residents with the same rights described above. If Arizona enacts additional privacy protections in the future, we will update this policy accordingly.

Arizona data breach notification

Under Arizona Revised Statutes § 18-552, we are required to notify Arizona residents in the event of a security breach affecting their personal information. Where required, notification will be made within 45 days of discovery of the breach, and to the Arizona Attorney General and consumer reporting agencies if the breach affects more than 1,000 Arizona residents.

Out-of-state visitors

If you are a resident of California, Colorado, Virginia, or another state with a comprehensive consumer privacy law, you may have additional rights under your state’s law (for example, the California Consumer Privacy Act as amended by CPRA). Contact us at the address in Section 09 to exercise those rights. As noted throughout this policy, we do not sell personal information.

07 · Data security & retention

Security

We use reasonable administrative, technical, and physical safeguards designed to protect the personal information we collect — including encryption in transit (HTTPS/TLS), access controls, employee training, and secure disposal of records. However, no method of transmission over the Internet or method of electronic storage is 100% secure, and we cannot guarantee absolute security.

Retention

We retain personal information only as long as needed for the purposes for which it was collected:

  • Contact form submissions — retained for up to 24 months, then deleted or anonymized;
  • Analytics data — aggregated and anonymized within 14 months;
  • Records required by law (for example, billing records under Medicare regulations) — retained for the legally required period.

08 · Children’s privacy

Our Site is not directed to children under the age of 13, and we do not knowingly collect personal information from children under 13 through this Site. If we learn we have inadvertently collected personal information from a child under 13, we will delete it promptly.

Parents or guardians who believe their child has provided personal information to us should contact us at info@azcareplus.com. This section aligns with the federal Children’s Online Privacy Protection Act (COPPA).

09 · Changes & contact

Changes to this policy

We may update this Privacy Policy from time to time. When we do, we will post the updated version here and revise the “Effective date” at the top of the page. If we make material changes affecting how we handle personal information, we will provide reasonable advance notice — typically at least 30 days before the changes take effect.
